Introduction
This privacy policy is to provide information to you, our client, on how your personal information (which includes your health information) is collected and used within our practice and the circumstance sin which we may share it with third parties.
Why and when your consent is necessary
When you register as a client of our practice, you provide consent for our health professionals, therapists and staff to access and use your personal information within the context of your therapeutic care. Only staff who need to see your personal information will have access to this. If we need to use your information for anything else, we will seek additional consent from you to do this.
Why do we collect, use, hold and share your personal information?
Our practice will need to collect your personal information to provide therapeutic and healthcare services to you. Our main purpose for collecting, using, holding and sharing your personal information is to manage your health and well-being. We also use it for directly related business processes, such as financial claims and payments, practice audits, accreditation, and case notes.
What personal information do we collect?
The information we will collect about you includes your:
Names, date of birth, addresses, contact details, employment details and other personal details
Medical information, including medical history, medication, allergies, adverse events, immunisations, social history, family history and risk factors
Medicare number (where available) for identification and claiming purposes

Dealing with us anonymously
You have a right to deal with us anonymously or under a pseudonym, alias or nickname unless it is impracticable for us to do so or unless we are required or authorised by law to only deal with identified individuals.
Our practice operates via telehealth (online) only. Therefore, all 50-minute sessions (excluding initial phone calls) require the use of video and audio. Clients are required to have working video and audio to participate in sessions.
How do we collect your personal information?
Our practice may collect your personal information in several different ways.
1. When you may your first appointment our practice will collect your personal information via your enquiry.
2. While providing health services, we may collect further information. At this practice we participate in the collection of health information you provide to us.
3. Within our practice, we utilise the collection of information facilitated by case notes. We complete case notes or session notes and store these in a secure way. Only your therapist has access to these, and should your information be shared with a colleague for training and professional development purposes, your personal details will be de-identified.
4. In some circumstances, personal information may be collected from a responsible family member, carer, colleague or employer.
5. We may also collect your personal information via phone or text, email or other contact methods of which you have engaged in.
When, why and with whom do we share your personal information?
We sometimes share your personal information:
With third parties who work within our practice for business purposes, such as accreditation agencies or information technology providers – these third parties are required to comply with APPs and this policy.
With other healthcare providers.
When it is required or authorised by law (e.g. court subpoenas).
When it is necessary to lessen or prevent a serious threat to a client’s life, health or safety or public health or safety, or it is impractical to obtain the patient’s consent.
To assist in locating a missing person.
To establish, exercise or defend an equitable claim.
For the purpose of a confidential dispute resolution process.
When there is a statutory requirement to share certain personal information.
When providing health services through My Health Record (e.g. via Shared Health Summary, Event Summary).
Only people who need to access your information will be able to do so. Other than while providing health services or as otherwise described in this policy, our practice will not share personal information with any third party without your consent.
Our practice will not use your personal information to market any of our goods or services without your consent. If you do consent, you may opt out of direct marketing at any time by notifying our practice in writing.
How do we store and protect your personal information?
Note here the different ways you store data and how it is kept protected e.g. paper records, electronic records, visual records (X-rays, CT scans, videos and photos), audio recordings.
Example:
Your personal information may be stored at our practice in various forms. Personal information is primarily stored in our clinical software system, which records client information, client notes, and booking details. Some information is also stored and backed-up on cloud-based document storage hosted on Australian servers in accordance with the APPs.
Our forms are generated using an online software system. Information collected is stored on an Australian server and complies with the APPs. Hard copies of records are also stored in locked filing cabinets and securely office stored whilst active. Our practice stores all personal information securely. We also use password protected computers, tablets, and smartphones. Access to personal information is only given to staff and contractors (such as our IT service provider) who require it to complete their job. Our staff induction process includes training on the protection of data, and staff and contractors are required to sign a confidentiality agreement to protect the data they have access to.
We use up-to-date virus protection software on our computers and regularly conduct software updates on our devices and website to help guard against cyber-attacks.
We store and dispose of personal information held within records in accordance with the Archives Act 1983 and relevant records authorities.
How can you access and correct your personal information at our practice?
You have the right to request access to, and correction of, your personal information. We require you to put this request in writing. Alternatively, you can request a copy of the form can be emailed to you. We will provide the requested information within 30 days of receiving your request via the x form. A fee of $40 will be charged for the preparation of this information.
Our practice will take reasonable steps to correct your personal information where the information is not accurate or up to date. From time to time, we will ask you to verify that your personal information held by our practice is correct and current. You may also request that we correct or update your information, and you should make such requests.
How can you lodge a privacy-related complaint, and how will the complaint be handled at our practice?
We take complaints and concerns regarding privacy seriously. You should express any privacy concerns you may have in writing via the below contact details. We will then attempt to resolve it in accordance with our resolution procedure.
